What to bring to your visit
- Passport or residence card
- Your insurance details (policy, insurer's contact information)
- Your medication record, or the medication you currently take
- Maternal and child health handbook (for children)
- Test results or medical certificates from Japan, if you have them
- A note summarizing how your symptoms have developed
